A Message from Nurse Erica

 

My nursing career began immediately after high school, evolving from CNA to MA, then to Registered Nurse, and eventually into nursing leadership. Early in my journey, I discovered my passion for pediatric nursing and have worked across pediatrics, PICU, pediatric oncology, and pediatric hospice care. Though I've explored various specialties including adult nursing, home health, and wound care (which I absolutely love!), nothing has fulfilled me quite like advocating for my fellow nurses.

The call to advocacy came when I first encountered the toxicity permeating our profession as a new graduate. I'll never forget sitting in my manager's office just weeks into my first RN position, being threatened with termination because I hadn't received enough compliment cards. My close nursing school friend Marlene experienced the identical conversation, which tragically led to a spiral of depression resulting in her taking her own life. That profound loss cemented my conviction that our profession desperately needed change.

Throughout my career, I've confronted bullying, harassment, unsafe working conditions, and organizational gaslighting. Despite being a single mother with everything to lose, I consistently spoke up against these injustices and reported wrongdoing. While I've certainly paid professional prices for this advocacy, I would make the same choices again without hesitation.

Leadership was never in my original career plan—in fact, I considered myself the least likely candidate for such a role. However, I quickly discovered that from a leadership position, I could effect meaningful change more effectively than as a staff nurse. During the early days of the COVID-19 pandemic, I posted a video about nursing leadership to TikTok that unexpectedly resonated with many, launching a new chapter in my advocacy work.

 

This led to me attending nursing strikes, content creating, and hosting this podcast, Nurses Uncorked. I've found my purpose in nationwide nurse advocacy and nurse-led legislation, dividing my time between working with legislators, unions, and supporting nurses in need.

In this episode of Nurses Uncorked, Nurse Jessica Sites and Nurse Erica interview Delanor Manson, the Chief Executive Officer of the Kentucky Nurses Association. They discuss the passing of House Bill 159 in Kentucky, which decriminalizes unintentional medical errors, making it the first and only state to do so. The bill provides immunity from criminal liability for healthcare workers and encourages a just culture of quality improvement and open communication. Delanor shares the process of writing and championing the bill. They also discuss the impact of the bill on nurse recruitment, retention, and the nursing profession as a whole. Additionally the discussion includes other legislation they have been directly involved in. Delanor highlights the Leadership and Advocacy Academy program offered by the Kentucky Nurses  Association to teach nurses advocacy skills and engage with legislators. They have implemented programs to encourage nurses to get involved in politics and decision-making.

In this episode, Nurse Jessica Sites and Nurse Erica discuss the worst Nurses Week gifts of 2024. They share examples of horrendous gifts, such as bubble wrap, a life-size cutout of Florence Nightingale, and a certificate for a free lunch at a restaurant two hours away. They reveal never before shared worst 2024 Nurses Week “gifts”. During this episode, Erica and Jessica share some of their own personal past Nurses Week gifts.  The nurses express their frustration with these thoughtless and insulting gifts. They also mention the importance of thoughtful and meaningful gifts, or which holds sentimental value. The nurses express their frustration with gifts that are patronizing or lack appreciation for their hard work.
 
Nurses Week should be a time to appreciate and celebrate nurses, but many hospitals give out thoughtless, insulting and often misogynistic gifts. In this episode, Nurse Jessica and Nurse Erica are joined by legal nurse consultant Matthew Garvey to discuss the recent cyber attack on Ascension Healthcare. The goal of this episode is to provide information and education for the frontline staff affected. They discuss the lack of preparedness by Ascension for such attacks and the potential liability that nurses may face. Board of Nursing liability still applies during a crisis, and nurses are accountable to the board's standards of practice. Suggested documentation should include a statement about practicing during the ransomware attack. Nurses should continue to enter incidents into the reporting system, despite any alleged discouragement. Lack of preparedness, such as not having sufficient downtime forms, binders or racks for paper documentation, is unacceptable and should be addressed by nurse leaders. There is a focus on the challenges of paper charting during a cyber attack and the lack of support from leadership. They also highlight the issue of managers not supporting staff during a crisis and the potential violation of their responsibilities. The incident command center is mentioned as a crucial resource that should be activated to provide support and guidance during emergencies. It emphasizes the need for clear and consistent messaging to build trust among healthcare workers. The discussion also touches on the ethical and legal considerations of informing patients about delays due to downtime. The lack of a bed board patient tracking system and the use of paper MARs are identified as major challenges during the cyber attack. Nurse Jessica and Nurse Erica spent their Nurses Week in Ohio attending the Rally for Ratios and Mental Health, organized by the family of Tristin Kate Smith ('Letter to my Abuser'). They express admiration for Tristin Kate Smith's family for their commitment to improving nursing conditions. They also mention media coverage and unpredictable weather at the rally. Jessica shares personal anecdotes about signs from loved ones in her life and at the rally. Nurses Erica and Jessica discuss their experience, their speeches and the impromptu march that followed. What a great way to wrap up Nurses Week!
 
In the second half of the episode, they talk about the challenges faced by nurses during the cyber attack on Ascension Healthcare, the largest Catholic hospital chain in the US. The attack has allegedly caused system failures, paper charting, and communication issues, allegedly putting patient safety at risk. Ascension's response to the attack has been criticized, and the nurses express their frustration with the lack of support and preparation.
